myMemorialChart Password Reset Instructions
Need help resetting your myMemorialChart login? Follow the step-by-step instructions below. Each step includes screenshots for guidance.
Step 1
On the Access & Signing Up page, under Access to myMemorialChart section, click Access the standard myMemorialChart portal.
A new tab will open, this is where you normally sign into myMemorialChart.
Under the Sign in button, select Forgot Password?
Note: This link is to reset your password only.
Step 2
Enter your current username and active email address.
Click the Reset Password button to continue.
Step 3
Check your email inbox for a message from mmhportal@mhsystem.org.
Open the email and click the link labeled Click here to sign in.
Step 4
A new browser tab will open, taking you back to the myMemorialChart login page.
- You will see your username is already entered and the password field is pre-populated: Do not change this; it is your current username and temporary password
- Do not allow your browser to save this temporary password
Click Sign in to proceed.
Step 5
The next page will look different. Here you’ll:
- Create a new password
- Answer your pre-selected security question
Please review the password requirements before making changes.
Once complete, you will be logged into your myMemorialChart patient portal.
