myMemorialChart Username Reset Instructions
Need help resetting your myMemorialChart login? Follow the step-by-step instructions below. Each step includes screenshots for guidance.
Step 1
On the Access & Signing Up page, under Access to myMemorialChart section, click Access the standard myMemorialChart portal.
A new tab will open, this is where you normally sign into myMemorialChart.
Under the Sign in button, select Forgot Username?
Step 2
Enter your active email address.
Note: This process resets both your username and password.
Click the Reset Credentials button to continue.
Step 3
Check your inbox for a message from mmhportal@mhsystem.org.
Open the email and click the link labeled Click here to sign in.
Step 4
A new browser tab will open to the myMemorialChart login page.
- You will see a string of random numbers in the Username field: Do not change this; it is your temporary username
- Do not allow your browser to save this temporary username
Click Sign in to proceed.
Step 5
The next page will look different. Here, you’ll:
- Create a new username and password
- Answer your pre-selected security question
Please review the listed username and password requirements before making changes.
Once completed, your will be logged into your myMemorialChart patient portal.
