Month of Giving Raffle Basket Donation

To Benefit Hunger Solutions – June 2025

We are hosting another basket raffle in support of Hunger Solutions during our annual Month of Giving! Help us raise funds to fight hunger in the communities we serve by having your department donate a basket for the raffle scheduled on June 23- 27. More details to come in Something’s Happening!

Commitments to donating a basket must be sent by end of day, Friday, May 16th. If you have questions, please reach out to Jamie Chen via Email (jachen@mhsystem.org).

Baskets and forms are due by Monday, June 16th, at 9 a.m.

Please fill out the form completely. Baskets must be dropped off/sent to:

• Sistersville General Hospital: administration office – c/o Lisa Edwards
