Journey Hospice

Journey Hospice is a small, not-for-profit hospice care provider for patients residing in Pleasants, Tyler, and Wetzel counties. Our primary team consists of a hospice medical director, who specializes in hospice and palliative medicine, certified hospice nurses (CHPN), social workers, a hospice coordinator, state-tested nursing assistants (aides), and a spiritual caregiver. Journey Hospice provides comprehensive end-of-life care in homes skilled nursing and assisted living facilities.

Journey Hospice provides 24/7 nursing availability, to ensure needs are met no matter the day or time. Care does not end when a patient passes away, but a robust bereavement program is initiated for the family. Our hospice coordinator and social worker lead our team to provide emotional support, bereavement groups, and a beautiful butterfly release memorial service for all the patients we care for. As patients may face different needs, Journey Hospice has a large, dedicated room at Sistersville Hospital that can accommodate our patient and their family members should they need a higher level of care while on hospice or provide monthly reoccurring respite relief for families as requested.

A hospice-eligible patient has an expected prognosis of 6 months or less, a patient is considered appropriate if their goals align with a desire to stop seeking aggressive inpatient treatment and or hospitalizations. Hospice provides comprehensive medical care at the bedside with an emphasis on comfort and a truly patient-centered approach the patient and family create a plan of care with the hospice team to meet the patients’ needs and provide the highest quality of life experience at home. Hospice provides medical equipment (hospital bed, bedside commode, oxygen, etc.) and medications that are needed for comfort such as pain medication, nausea, and antibiotics, at no cost to any patient who has Medicare Part A. The best part of Hospice is that this complex level of care is 100% covered by Medicare Part A and most insurance provides the hospice benefit at no cost to patients.

Our highly qualified and trained professionals provide quality compassionate care and comprehensive services to all ages.

Our regular office hours are 8 a.m. – 4:30 p.m., Monday through Friday. Nurses are on call 24 hours a day, seven days a week at both locations.
