Our primary care providers are part of the same health system as our specialists, so access to your medical records is seamless and easy for every health care provider you see. myMemorialChart offers personalized and secure online access to portions of your electronic medical record (EMR), where your provider maintains your health information. It enables you to securely manage and receive information over the internet, message your provider, get prescription refills, and more—wherever and whenever you need! Our providers value the importance of connected care and are dedicated to providing a full continuum of care to every patient. In fact, if you have an inpatient stay at one of our hospitals, you will most likely have a follow-up appointment with one of our primary care providers. This ensures that you are healing so you can get back to being yourself…but healthier!
Our Family Medicine Residency Clinic
Conveniently located on Belpre Medical Campus, our Family Medicine Residency clinic houses our current practicing residents and their teaching physicians. These residents are physicians who have graduated from medical school and are training to specialize in family medicine, also called primary care. They train for three years with an experienced, board-certified teaching physician, honing their skills to care for pediatric, adolescent, adult, and geriatric patients.
Teaching physicians review and supervise all patient visits conducted by the residents. During the first six months of the residency, the teaching physician participates in the patient visit. Therefore, the resident physician’s care is covered by the same insurances as their supervising physician’s. Telehealth
Beginning August 7th, 2023, all video Telehealth appointments will be completed in the myMemorialChart application. To be able to participate via video you will need to be enrolled in the myMemorialChart. If you have not signed up for this, please visit mhsystem.org/myMemorialChart or download the Meditech MHealth App.

- Reducing the risk of spreading or contracting contagious disease
Instructions for downloading MHealth App:
From your Apple/Android or Windows Smartphone device
- Go to the app store on your phone
- Search for the MEDITECH MHealth app with a green heart icon
- Download the MEDITECH MHealth app
- Choose Memorial Health System MyMemorialChart
Preparing for your visit:
- We recommend you find a quiet, well-lit space in your home.
- We recommend using a broadband internet connection or cell phone with a minimum of two bars.
- We recommend you wear comfortable clothes that will allow them to show their provider areas on the body they will need to assess.
- We also recommend you log into the telehealth application 10-15 minutes prior to the visit to test out the sound and camera on their device.
- To prepare for the visit, please have this information available:
- Have a complete medication list available or medication bottles available during the visit.
- Write down your list of questions for the provider.
- Obtain basic vital signs (weight, heart rate, respiratory rate) prior to the visit.
